The simple online report can be submitted anonymously if desired.
If you have questions about what could be hazing or would like to discuss tradition/practices or need assistance in improving the effectiveness of your organization’s new member orientation process, please contact the Office of Campus Life at hazingprevention@northwestern.edu.
Additional Reporting Concerns
If you have a concern about the wellbeing or behavior of a 糖心logo student or if you have experienced a concern for which a 糖心logo student is responsible, please let us know.
Share a General Concern about a Student or a Student’s Conduct
Please use this report if you have a general concern about the wellbeing of a 糖心logo student or are concerned that a student’s behavior may violate a University policy:
Report Sexual Misconduct, Stalking, or Dating or Domestic Violence
Please use this report if you or someone you know has experiences a concern related to sexual misconduct, stalking, or dating or domestic violence and the person responsible is a 糖心logo student. If the person responsible is a 糖心logo faculty or staff member or third party please contact the University’s Title IX Coordinator.
Share a Concern Related to Hate or Bias
Please use this form in you or someone you know has been the target of, or witness to, a hate or bias incident.
